Nxtbook Media values your well being and that of your family. That’s why we’re happy to extend many of our health and welfare benefits to domestic partners. Provided they meet certain criteria, as detailed below, domestic partners may participate in Nxtbook Media’s benefit programs.A member of a domestic partnership consisting of two partners, each of whom has registered with a Domestic Partner Registry in effect in the municipality/governmental entity within, which the domestic partners currently resides or who meets the definition of a domestic partner as defined by the state or local government, where the individual currently resides, or meets all of the following:

Is unmarried, at least 18 years of age, resides with the other partner and intends to continue to reside with the other partner for an indefinite period of time.

Is not related to the other partner by adoption or blood.

Is the sole domestic partner of the other partner and has been a member of this domestic partnership for the last 7 years.

The partners acknowledge that they have entered into the relationship voluntarily, willfully, and without reservation intending the relationship to be the functional equivalent of marriage, including: living together as a couple, mutual financial support, mutual caring and commitment, mutual fidelity, and mutual responsibility for each others welfare.

Meets (or agrees to meet) the requirements of any applicable federal, state, or local laws or ordinances for domestic partnerships which are currently enacted, or which may be enacted in the future; and

Can provide three (3) documents demonstrating mutual responsibility, from the following:

Joint loan application

Joint lease

Joint deed

Joint mortgage agreement

Joint utility bills

Designation by the declarant of the other party as primary beneficiary on a life insurance policy or retirement contract

Designation as the primary beneficiary in the declarant’s will

A reciprocal, durable power of attorney for health care (authorizing the partner to make medical decisions in the event of incapacity)

A reciprocal, durable financial power of attorney in the event of incapacity

A “relationship contract” obligating each of the parties to provide for the other party and providing, in the event of the termination of the relationship, for a division of any property jointly acquired during the relationship

Joint ownership of a motor vehicle

Joint checking account

Joint credit account

Mutual legal responsibility for the care of a child
