The name on your FPGEC application must match the name that appears on your official transcripts, license/registration, TOEFL iBT report, and the primary identification that will be used to be admitted to the FPGEE. If your name has changed, you must send a properly certified copy of a marriage license or other legal document, such as a court order, that indicates the full former and current names.
If you use your father’s and/or grandfather’s name(s) or patronymic names as part of your name as shown on your transcripts, please enter them in the space provided in the online application for middle names. The family, tribe, or surname is to be indicated in the space provided for “last name.”
If your name has changed on the primary ID approved by the FPGEC, since applying to the FPGEC, you must ensure that there is enough time to update your name with NABP prior to taking the FPGEE. Supporting documentation for the name change must be submitted to the FPGEC no later than 30 days before your examination appointment. See “Bring Your ID” and “ID Requirements” in Section 13 for important information on ID name matching requirements. Questions on this process may be sent to FP@nabp.pharmacy.
IDs such as a driver’s license, Social Security card, or permanent resident card cannot be considered as legal documentation of a name change. Be sure that the information you submit on the application form matches the information on your transcript.
